Document Download: End of Year Inclusion Report Template

The 'End of Year Report' Template provides you with an example of possible content that can be used to provide families with an annual statement of progress, success, challenges and next steps.  An important peice of communication that informs parents, enables collaboration with the team and provides future placements of the current foci in your setting.

I would use this document set up as a 'mailing merge' document.  Creating an excel spreadsheet for each team member to contribute to which can simply and quickly merged into single reports for each family.  

An "External Agency" could be a company or organisation such as Educational Psychologist, Medical Doctor, Speech and Language or other professional requiring school data and can also contribute if they are not providing their own report.
