Document Download: Confidentiality Agreement

The 'Confidentiality Agreement' is a one page document issued by school for signature by External Agencies. A school should use this document to seek confirmation that an External Agency to school will not release a learner's private and confidential information to any other person or organisation. The school should keep this document on file.

An "External Agency" could be a company or organisation such as Educational Psychologist, Medical Doctor, Speech and Language or other professional requiring school data.
